This soundless video shows removal of multiple cystic lesions consistent with neurocysticercosis. The patient is a 23-year-old man who had emigrated from Mexico and presented with worsening headaches and vomiting. The cysts causing brainstem compression are resected via a supracerebellar infratentorial approach with the patient in a sitting position. Gliotic tissue to the left of the midline is opened, and a neurocysticercosis cyst spontaneously evacuates. Additional cysts are resected using suction and rongeurs. After completion of cyst evacuation, the cavity was closely inspected. No additional cysts were identified. Contributed by Frank S. Bishop, MD, and William T.
